Waiver, Assumption of Risk & Agreement to Hold Harmless
In consideration of acceptance by THE DOG TRAINING CLUB OF, CHAMPAIGN URBANA INC. (the “Club”) as a member of its dog training classes of instruction, I (we) understand that attendance in a dog training class, informal dog training not related to any scheduled class, and participation in any event at the DTCCU training building, is not without risk to myself, members of my family or guests who might attend, or my dog, because some of the dogs to which I will be exposed may be difficult to control and may be the cause of injury even when handled with the greatest amount of care.
I hereby waive and release the Club, its officers, members and agents from any and all liability of any nature, for any accident, injury, damage or illness which I, my dog, my family or guests may suffer. This includes specifically, but without limitation to, any accident, injury, damage or illness resulting from the action of or exposure to any dog, including my own; and I expressly assume the risk of any such accident, injury, damage or illness while attending any training session or other function of the Club, or while on the training grounds or surrounding area thereto.
In consideration of and as inducement to the acceptance of my application for training by the Club, I hereby agree to indemnify and hold harmless the Club, its officers, members and agents from any and all claims, or claims by any member of my family or any other person accompanying me to any training session or function at the Club, or while on the training grounds or surrounding area thereto as a result of any action by or exposure to any dog, including my own.
I understand that I and/or my dog may be photographed, filmed or videotaped at a Club class or event. I hereby irrevocably grant to the Club, its officers, members and agents the absolute right and permission to use my and/or my dog’s likeness and/or voice for any purpose whatsoever, including commercial advertising. I hereby release, discharge and agree to save harmless the Club, its officers, members and agents and all persons acting under its permission or upon its authority or for whom it is acting, from any liability by virtue of any publication of my likeness, including, without limitation, claims for libel or invasion of privacy.
